My understanding is case series is descriptive pattern for a group of patients who developed the same disease or variant of it! can we include different outcomes of the same procedure in one case series? (aka, leak and reflux are both complications of sleeve gastrectomy, can we include patents of both disease in one series?

I think you can report success or failure of a procedure using different types of cases with emphasis on the procedure.
Yes, it would be perfectly acceptable to include a description of the various outcomes, or complications, that result from a single procedure performed on several patients. As you indicated, a case series is used to describe a group if indiduals with the same disease, or disease variant, but also that have undergone the same procedure. The outcome is not the disease or common thread. In your example, the sleeve gastrectomy is the common thread, and then it is appropriate to describe the outcomes and complicatipons observed in that population.
Yes, it would be very reasonable to include different outcomes for a particular procedure. The procedure or therapy in this case would be the area of interest, and all relevant scientific information including outcomes would be appropriate to include in the series.
