My experience is that the journal choice has to be even more specific. You say "Medicine" but, as you know, there are many specialties and many open access and/or free ones in each specialty. It is important to find one that is suited to your topic to get your work published. If you choose a journal with different aims or scope or interest, they might reject you just for that even if your work is good already.

It may be helpful to consult the Directory of Open Access Journals (DOAJ), www.doaj.org where you will be able to do a journal search limited to Medicine and a few specialties within medicine that the DOAJ categorizes journals into.
