We are working on a study focused on adolescents' transition to adult care and are looking for a validated measure for satisfaction with the transition. So far the only measure that we have came across seems to be the "Mind the Gap" scale; however, 1) we have not been able to clearly identify how and when the tool is administered and 2) if it is an appropriate tool to measure satisfaction with transition of care as the majority of the items seem to be focused on the currently receiving pediatric care (so more of a satisfaction with care rather than satisfaction with transitioning to adult care).

I would appreciate any feedback and recommendations for other potential tools.
