In response to your question, I should say that, as you know, in most of cases, we do not unfortunately have access to standard questionnaire fit with the subject of our research. That is, we have to design a researcher-made questionnaire which can serve the objectives of our research. Specifically, concerning your question, I would like to respectfully recommend you that, if there is not any standard questionnaire related to your research, you will have to design it. For this purpose, first, you have to find all indexes which are significantly as well as correctly characterizing the input/output variables defined by you in your research. To this end, you could helpfully benefit from the available relevant literature as well as interviewing with elites known in the main area of your research. After that, you could make statements which address the effect of each of the indexes related to the input variable (s) on every indexes defined for the output variable (s). These statements could be inquired in your questionnaire and, hence, you could find all your required raw information and data through them.
