The United Stated Census Bureau collects data from the elderly on this subject. The data is categorized as "assistance with daily living". No one thinks that the data is adequate. I remember that it is limited to mobility and selfcare. Also, JAMA-Journal of American Medical Association has published a couple of articles on the subject. You may want to explore their questionnaire.
