The alcohol can be of phenolic or alkyl type and the RNH3+ as part of an amino acid side chain.

No reaction will take place between R-NH2 and R1OH under physiological condition. Both -NH2 and the -OH groups are nucleophilic in nature and also are bad leaving group in a substitution reaction. Unless one is converted into a good leaving group (say by protonation) reaction is most unlikely

Possibly RNH3+ + ROH = RNH2 + ROH2+.Proton transfer between the amino and alcohol groups which should be reversible at physiological pH
