Could anyone suggest any web server/software/ any other method which identifies or lists all the salt bridges and hydrogen bonds present in a protein structure.

You may use the VMD visualization package for the analysis of salt bridges and hydrogen bonds present in a protein structure.
First load your protein, then
For salt bridge interactions, Extension < Analysis < Salt Bridges
For hydrogen bond, Extension < Analysis < Hydrogen Bonds...
Along with this you may also use visualizing softwares like "Pymol" and "Chimera".
