We need a universal portal with labs and other test results available for all providers to access. There is no continuity of care and some people are on 5 different medical portals!!!

--Expensive tests are repeated because no one knows about them, doesn't have time to fill out forms to get a copy of them, and trying to get results from another system takes too long.

--Abnormal tests are not known about by specialists and other providers and no one knows who will treat that abnormality half the time.

One system will let all providers know how a patient is doing from a holistic standpoint, allergies and current medications will be checked and verified every visit by every provider so patients don't have to rewrite everything on every portal!!! This should lessen mistakes and oversights.

@kellygardiner thank you for your thorough insight. Very detailed and experience-based answer! Over-testing by physicians and a lack of coordination when it comes to patient diagnoses is something I have experienced first hand. I will make sure to add this to our project.

best evidence and empathy

The biggest concern I have is with the transparency of medical information crossing from one system to another. We are still struggling with organizations that hold information back if clients are not staying in the same system due to the inability to access parts of the record. Transparency of digital images and reports result in duplicate orders since the client has to have procedures redone to allow for new providers to have a full understanding of the condition or situation.

It also puts the patient in a situation of having to ensure that they have access to all their portals to be able to share their information. This is not a viable path when they are not feeling well or not interested in providing this information.
