There is a questionnaire used for assessing Health related Quality of Life (HRQoL) after critical illness named "SF-36" .
It was mentioned that:
" ..... All but one of the 36 items (self‐reported health transition) is used to
score the eight SF‐36 scales....."
Anyone who knows about this questionnaire, what does this sentence mean ?

The Short Form-36 (SF-36) is a 36 item questionnaire which measures Quality of Life (QoL) across eight domains. They are both physically and emotionally based. The eight domains that the SF-36 measures are as follows: physical functioning; role limitations due to physical health; role limitations due to emotional problems; energy/fatigue; emotional well-being; social functioning; pain; general health. A single item is also included that identifies perceived change in health, making the SF-36 a useful indicator for change in QoL over time and treatment. It is free to use. Over 5000 published studies have been carried out using the SF-36, demonstrating its abilities as a global QoL Measure (applicable to many conditions and professions).

SF-36 can assess the various dimensions of Quality of Life. The only challenge comes up in the analysis and interpretation of the results. Requires a good background in statistical modeling else you may need to seek the help of a statistician.
I think that the other challange is concerned with the Cross-Cultural adoptation and translation of this tool. Yesterday I came across a very good recent literature review around this :

I have used the SF-36 previously in a cancer patient population. From the SF-36 you can also calculate an overall Physical health Component Score and Mental health Component Score. Depending on the population you wish to administer the questionnaire to, you should be wary that older people or people who are not in work may omit some items as some questions are related to functional ability in work. I am working in Oncology center in Jordan soI was interested to measure QOL of these population however i feel that most of the patients entered to the ICU die in the same admission or after a very short while after discharge.
So if the plan is to follow oncology patients after discharge for 6 months and one year, i am afraid that it will be challenging to have a good sample of those patients.
