You can check also this questionnaire witch developed by (Wu, Taylor, & Chen, 2001) on the link below, it contain the five dimensions PD,UA,MAS,IND,LTO with a scale of 5 degree from Totally disagree to totally agree.
It seems to be difficult to locate the original Hofstede questionnaires which are probably archived and protected; but they must have been likert scale because the testing involved scale proximity of tolerance toward a behavioral/societal dimension or value. The position of tolerance (toward a dimension like masculinity or uncertainty avoidance) was unique because it produced results that defied stereotype and hypotheses in that it did not directly ask agree, disagree but rather a sense of contentment with how things are.
