About "alexithymia" as cognitive functional disorder of the brain?

We suppose "alexithymia" is the cognitive functional disorder of the brain.

Do you think that, patient can express the symptom in conscious (optionally)? For example, it depends on the other party facing each other. But when we suppose that, the patient is after developmental age (adult).

And my question is, for example, Patients side able to control. They judge a partner ( object ) , and control an expression of the athymia. For example, a deadpan.
