Is there a lower cut off of neurological deficit in ischemic stroke which would make you reluctant to do iv-thrombolysis knowing the risk of ICH against the possible benefit (NIHSS of 3?)?
NIHSS score should not be the only factor for the clinical decision. For example, isolated aphasia can cause a serious disability with a lower NIHSS. Other variables to consider might be time-to-needle, imaging and the patient's general status. Therefore, in my opinion, a lower NIHSS score should call for caution without constituting a strict cut off.
