A male patient, aged 45 years old, farmer, non-smoker. Complaining of dyspnaea on exertion for 3 years, cough with slight expectoration, and precordial pain. Splenectomy was done 4 years ago.

On examination, HR was 90 b/m, regular, BP 110/70, no cyanosis or clubbing of fingers. There was a pan systolic murmur over the tricuspid area and the apex, with early diastolic murmur over the pulmonary area. The chest was clear.
