The drug combination mostly used in our environment for the management of COVID -19 is the combination of hydrochloroquine, azithromycin, zinc and vitamin c.
Zinc Sulphate and Vitamin C are supplements to boost the immunity. Zinc and Ascorbic acid plays a vital role in effectiveness of reticulo endothelial system. Hydroxychloroquine ease out the air-way obstruction. Evaluate the ECG especially RT before prescribing. Hydroxychloroquine is not for prophylactic use.
Very good question. It is not only to boost the immunity. Zinc is crucial in many signaling pathways. To answer the question we need additional information from in vitro studies> In exemple, I work on a HCQ-modified molecule with a Co and we should know more soon> I will post the results when data are concrete.
Recent studies indicate that hydrochloroquin given alone has more harm than benefits in Covid-19 patients.
Vitamin C is a critical factor in membrane repair process of cells, including airway and gut epithelia.
Zinc: I have done some research with zinc about 10 years ago. My results showed that zinc is an effective blocker of a large conductance calcium-activated chloride current extensively expressed airway and gut secretory epithelia, as well as blood vessels-Cells that are primarily effected by the coronavirus that causes Covid-19. I think zinc block of this channel may benefit in Covid-19 by reducing chloride secretion, subsequently resulting in reduction mucus output that can block the airways and lungs.
For further details and references, please see my publication listed below. Unfortunately in a corpoarte world, my study received very little attention and I received no funding to continue my research.
Thanks @Refik for your valuable comments. Your paper was very informative. However, the recent evidence on chloroquine may appear premature to some of hs who have used it for decades to treat malaria without much problem. We use the combination in the treatment of COVID-19 in our environment and more than fifty percent of our patients have been discharged after becoming Negative. Perhaps genetics and environmental factors are at play.
Briefly,Zinc has penetrating ability which boost anti micro activity of chloroquine, it also helps immunity maintenance , VitaminC, also helps a lot in boosting patient's immunity which is one of special weapon in combating COVID-19
Zink has been shown to have important role in immune boosting. Importantly, Zink has an effective antiviral response. It was further shown in vitro that zinc inhibited coronavirus RdRp activity and that zinc ionophores blocked coronavirus replication. refer :https://doi.org/10.1016/j.mehy.2020.109815
Zinc acts as an antiviral, and once inside the cell is thought to neutralize the virus. Chloroquine softens the cell membrane and acts as a zinc ionophore, binding to zinc ions and carrying them into the cell where it then stops viral replication via RNA polymerase.
Below are a few articles that may give more understanding to this. The last report is regarding HIV and how zinc added to therapy improved outcomes greatly with suppression of the opportunistic organism, pneumocystis carinii, particularly noted.
There are also some excellent videos regarding zinc and hydroxychloroquine on a medical university teaching website that is free on covid-19, and taught by a molecular biologist and pulmonary intensivist at medcram.com. #34 is real good one.
Maria Eugenia Barrientos).
Focusing on the Covid19: The Vit. C and Zn are an excellent duo as antioxidants and immune system boosters. Regarding Azithromycin, it is undoubtedly the most effective antibiotic for different infectious conditions, including respiratory conditions. In the initial (symptomatic) phase of Covid19, the combination of all 3 is likely to work well, replacing Hydroxychloroquine with Ibuprofen (this NSAID is effective in the therapy of Covid19, which is internationally prescribed by the prestigious Dr. Maria Eugenia Barrientos
Addition of Zinc sulfate and Vitamin C along with Hydrochloroquin is a very wise step taken for Covid-19 patients treatment. Zinc sulfate important for nerve function to revive the sense of taste & smell which is lost in Covid-19 patients. Vitamin C as an antioxidant is beneficial as a scavenger of free radicals and thus would be effective in preventing the organ damage from free radicals.
Some drugs (like hydrochloroquine) have capabilities to bind with metals phenomenon called as Metal-ionophores. Zinc is given to patients because it alleviates respiratory tract infections as reduce viral activities and also contributes in an immunity enhancement. Likewise Vitamin C is given to patients to boost their immunity.
