The effect of cognitive bias in medical diagnosis needs extensive study to reduce the incidence of medical errors.With the focus on specialization practice ,the concept of seeing the patient in total is loosing .The incidence of  heart failure in patients diagnosed with copd is 25% as per an evidence based medical textbook.A systematic audit will probably tell the areas of cognitive bias in medical diagnosis and hence reduce medical errors.

Cognitive errors in medicine are a huge problem. Overconfidence/excessive certainty, under-appreciating the importance of the base rate, anchoring of diagnoses, and others are all significant. I've just submitted an article about overconfidence in diagnosis - I can send you a copy if you like.
