A 65 years old diabetic and hypertensive patient with post lacunar infact 5 years ago now presents with persistent stiffness, heaviness on the affected side ( both upper and lower limbs) no other symptoms.
stiffness occurs all diabetic patients due to collagen glycosylation. this stiffness depends on post lacunar infarct? what does it mean, is it a kind of osteomyelitis?
This was the original CT scan of the patient back in 2011 l where after a small hemorrhage in the lenticular region a diagnosis of lacunar infarct is being made . Now gradually overtime the patient complaints of heaviness in the lower limb specifically which is more worse in the morning. He is unable to lift his left and bend it at the knee. However this is not a fixed flexure as he is engages in his daily routine activities. My question is is there a pharmacological agent which can improve these symptoms of stiffness and feeling of heaviness ?
May I know his functional status, A1c, & BP control now? Poor mobility with autonomic neuropathy (that may lead to alteration in blood flow dynamics in affected limb) may cause edema during night that may make limb feel heavier by the morning. I think physiotherapy, good metabolic control and checking his Vitamin D and B12 might help.
