cavity lining should be given in anterior teeth also, mainly based on the depth of the cavity and clinician choice. 

A good restoration technique with an effective adhesive system

if we want a material for pulp caping the MTAis a good choice.

The indirect pulp capping in the anterior teeth should be done in a similar way that in the posterior teeth, with some cautions. The material that should be chosen is the glass ionomer cement. This material can promote a suitable sealing with slight inflammation, observed in just a few days after application, without further consequences.

These materials present good biocompatibility, reduced solubility and great compressive strength (compared to the other cements). But you should be careful to work with these cements in anterior teeth, or any other material used as a mechanical barrier, since thick materials can promote a shadow and compromise the esthetic of the restoration. Apply just a lining to avoid the penetration of the monomers on the dental pulp space.

The self-etching adhesives can be applied in deep cavities (not too close to the pulp tissue), in situations that you guarantee that do not have any direct contact with the dental pulp. In cases that probably have a reduced communication, almost imperceptible, both glass ionomer and self-etching adhesives should be avoided.

Biodentine™ is ideally suited for use as dentine restoration in deep or large carious lesions. It is a calcium silicate based material with good mechanical properties and excellent biocompatibility.

Use of non-eugenol based lining materials is advised especially in conjunction with resin composite restorative materials.

Has the famous lining material calcium hydroxide (Dycal) been abandoned? 

If the seal is the seal, why a special lining material has to be used anyway?

Only when a direct contact to the pulp exists (exposure), there is the struggle to keep the pulp vital, Calciumhydroxide (still gold standard for direct capping according to syst review by T Hilton in Op Dent 2009) is not able to do that very succesfully and in a recent RCT (Hilton, 2013, J Dent Res) MTA has proven to be superior in that respect. As an MTA-related material BIODENTINE might be a good choice to as the handling is better then MTA. But clinical evidence for that is not yet available.

For deep cavities, a liner is not needed. Larger volumes of glass-ionomer might compromise the strength of the restorative tooth complex and result in more fracture on the long term. Just use a gold standard adhesive: either an etch and rinse three step like Optibond FL or a two step self etching system like SEBond.

I never use base under composite even in deep cavities. Just self-etching bonds.

The goal for a permanent tooth with deep caries and the potential for exposure of pulp that is vital, asymptomatic, and exhibits no radiographic evidence of periapical infection is to maintain pulp vitality. An indirect pulp cap is preferable.

According to your comments above, I think it can be concluded that the best material to use in deep cavities as a lining material underneath composite is:

1. MTA as it helps in maintaining pulp vitality.

2. Biodentine is a good material; however no enough scientific evidence is available.

3. Calcium hydroxide is a good material but less effective than MTA.

Although I have am unable to quote any studies, I have personal experience (a deep caries lesion in my own tooth) that light-cured hydroxyapatite can work well in managing deep caries. Since this material need be placed only on the area closest to and at danger of exposing the pulp, it is unlikely to affect the esthetics. Over this any etch-bond system can be applied. Biodentine also has shown good results in many cases.
