Am originaly from South Africa, truelly speaking I cant remember the Nurses having my records. As you are stating, one has keep on Recordbook, which unfortunately gets lost sometimes. Private Doctors or Hospitals do keep records but Goverment clinics especially in the rural Areas, dont think so.
