Can anyone please tell me the database names or websites from where I can download human SNP datasets along with the quantitative traits (phenotypes) for genome-wide association studies (GWAS)?

you can check the GWAS catalogue at : https://www.ebi.ac.uk/gwas/
You will have to validate the database for your purpose. The NHGRI-EBI Catalog, for example, does not include earlier studies in Alzheimer's disease GWAS.
You can get both genotypes and phenotype data from dbGaP (https://www.ncbi.nlm.nih.gov/gap/) for the GWAS study.@Jahangir
