I pressume that other FEM packages are able to solve acoustic problems as well.

As for Matlab, you can use tools from Partial Differential Equation Toolbox. However, there is GUI for wave propagation in 2D space only. If you want to solve 3D problems, you have to do it programmatically.

Perhaps Code Tympan which is an open source code for sound propagation suit your needs? I have never used it but I believe that it does cater to some degree for non-straight sound transfer paths. You find the code and a manual here. https://bitbucket.org/TYMPAN/code_tympan

Code Tympan is scalable up to higher frequency as it is based on raytracing.

I agree with Luboš that you should consider FE if you wish to go more hardcore physics and mostly focus on low frequency. Not as ready made as Ansys, but highly tweakable as it executes inside Python would be Code Aster (CA).

CA is quite a clever code. Inhomogeneous acoustic data is not a problem, but it does not cater for wind effects in its acoustic simulations. That said if wind matters, you can either make your own element as CA is open source or build the system matrix and then add the compensation.

For the ground - if you are into air-soil interaction - CA comes with a set of soil models.
