The second option, SurveyMonkey, could easily be the best option if you have the ability and willingness to go for the paid version. Most of my acquaintances tell me that SurveyMonkey is a reliable option. One of my friend has informed me that its results are reliable and it is worth investing in the gold version because even the responses are downloadable in SPSS format very suitable for further analysis.
Another good option is Zoomerang but has been considered less user friendly than the discussed other options.
Also, Google Forms is a good option if your sample size is small and the number of questions are limited. Further this is a free tool .
Then there is option of uploading your surveys on your own institution’s website and sending the link to the respondents if at all there are people in your IT department who can design it for you.
I am not aware of any such tool but i think you can talk to the IT guy in the department and i think he can connect a timer with the questionnaire and hoist the questionnaire on the website of the university. Then you can send the link to the respondents. Theoretically seems possible to me, let's speak to someone who knows programming to see if it is doable. if i find out i will revert back.
