Dear Alexander, I totally switched to bipolar resection in 2009 after changing the equipment. The advantage in comparison to monopolar TURP is the avoidance of TUR-Syndrom. With modern instruments it is possible to operate as quick as with monopolar technique. In TURB the incidence of N. Obturatorius reflex is much lower. So I do not see an indication for monopolar TUR any more. Best regards
At my institution we have also switched almost entirely to bipolar TUR-B and P. I do not see any advantage with monopolar. If you use the mushroom for the prostate you will not get any tissue, which is relevant in a few cases.
According to my experience, if we analyse the resected mass of the prostate there is stil advantage of the monopolar TURP. Of course new generations of the bipolar instruments could change it.

Bipolar TURP give the chance to use Normal Saline, which decrease the risk of TURP SYNDROME, and decrease the risk of obturator reflex, but do not decrease the risk of other complications, if it didn't increased as Dr. Bijan said.
I simply enjoy doing bipolar resection. In my experience; it is associated with less complications including less bleeding and theoritically Lower risk of TUR syndrome. I wrote theoretical as I still also do mono polar resection and working fast we do not really encounter TUR syndrome. It is a rare complication. The only advantage that makes me still using monopolar is being cheaper so it is valid option for patients with small prostate with no medical risk factor.
