You are looking for a very difficult item. Often times, productivity is measured in terms of employee output (units produced), sales amount in dollars, etc. The most common approach has been to produce an index of output variables.

Measuring employee productivity is much more involved than simply a Likert scale that goes, let's say from 1 to 5. Hernan mentioned outputs which would place the levels of measurement into a specific scale. You may want to build a questionnaire around scales instead of the Likert template.

You have to be aware that there are "hard" measurements of productivity like hours per product, products per hour, average service time, sales per day, and so on. Thus, if you are opting for a Likert Scale type of productivity measurement, you'll be using "soft" productivity measurements, which deal mostly with the perceptions of such productivity. Also, a "harder" measurement of productivity would be the performance appraisal records.
