A patient was given 300 mg Theophylline taken twice a day for the treatment of her bronchial asthma and has a positive clinical response. She has been compliant in taking her medication however, she has complained of tremor, palpitation and, anxiety. Her plasma theophylline level was measured at 7 mg/l which is within the acceptable therapeutic range and outside the toxic levels.

Theophylline, theobromine and caffeine are in a chemical class that is referred to as the methylxanthines. Does she drink tea (contains theobromine), coffee (contains caffeine), soda pop (some may contain caffeine) of some other similar product? If so, the effects may be additive and she may need to cut down her theophylline dose. Perhaps, to a single dose per day. She should probably see a physician (M.D.) to help with choice of drug or dosing. A clinical laboratory might be able to measure total methyxanthines.

I share the opinion of Tomasz Grabowski that side effects reported in this clinical case are well known with theophylline as you can see on this page: "https://www.medicines.org.uk/emc/product/1021/smpc#gref".
However, the simultaneous use of other drugs that may cause these types of adverse effects or the existence of other disorder
s (e.g. ionic, which may explain palpitations) increased by taking theophylline must be taken into consideration.
