I would recommend Maria to use Hussin's questionnaire. It,s a good and a complete one. However, I'd suggest that  the style of teaching (Item 1) be a title of a rubric questions. Because a teacher may use a magistral, explicative, participative  or a mixte, style of teaching all of which the following questions are based on.

I doubt if the questionnaires that you seek may meet your objectives because the context and learning environments could differ. You could research on the framework used for educational surveys in the U S, Documenting Excellent Educational Practices (DEEP).
