Personally, I don't use Simulink to simulate motor dynamics, but your query is more of a conceptual nature.
You seem to have entered parameters in the block mask correctly. Note that there are separate columns for Lls and Llr, the "second l" in each case indicates leakage. Also there is a separate column for the magnetising inductance Lm, so this quantity is obviously not included in the other two parameters.
In the table provided by you separately on the other hand, both Ls and Lr are "large" values, so that if you obtain ratios Xs/Rs and Xr/Rr, then these are of the order of 100 or so - obviously, Lm is included in both by the respective leakages. That is:
Ls = Lm + Lls
Lr = Lm + Llr

It is always a bit tricky to ask others - particularly when there is stuff like fuzzy logic involved !!
