For the Philippines, I would suggest Phil Spradbery's "A Manual for the Diagnosis of Screw-worm Fly" http://www.animalhealthaustralia.com.au/wp-content/uploads/2011/04/Manual-for-the-Diagnosis-of-Screw-Worm-Fly.pdf
I think as a general text (although a bit out of date) one of the most useful wide ranging texts is Maurice T James book "The flies that cause myiasis in man". You can preview it at http://books.google.co.uk/books?id=yCsuAAAAYAAJ&printsec=frontcover&source=gbs_ge_summary_r&cad=0#v=onepage&q&f=false.
Alternatively Fritz Zumpt's book "Myiasis in Man and Animals in the Old World: A Textbook for Physicians, Veterinarians, and Zoologists".
Both need to be checked for updating of species names and also species recently recognised but for basic field/lab work they are excellent.
Diagnosis of myiasis is not so difficult as such. External larvae could be visible on open wounds or located in subcutaneous nodules. In the later situation, larvae should be collected and identified . Larvae could be of various developmental stages. However, species identification could be challenging. An entomologist might be required for this task. An alternative option is to collect mature 3rd stage larvae and allow them to produce the adults. Identification of adults is more easy.
