Generally, 'packaged' questionnaires are of very limited value, except possibly for very linmited and general surveys of attitudes or perceptions. Even then, you need to know how to correctly interpret the results and obtain sufficient responses.

I highly recommend getting a free (limited) subscription to SurveyMonkey - https://www.surveymonkey.com/welcome/sem/ - for both excellent advice on developing questionnaires and interpreting them. They may also be used to collect the data if your respondents have access to the Internet.
As with all research and investigations, you really need to know what you are specifically looking for before you can devise an experiment to get the data that you may need. I do that sort of thing as a consultant, but if it is for an academic program I think that is part of the learning process.
