In general PIN diodes/Varactors are simulated in EM simulation software (HFSS/CST) using lumped RLC boundary. But is there is a better way of simulating active devices?
Can I include SPICE definitions in a EM simulations?
Consider design of an antenna that uses varactor for tuning. How do I plot the radiation patterns for different biasing voltages?
You can define the applied voltage as a variable with some initial value, simulate the design and then tune the biasing voltage to observe the effect. I have not tried including SPICE definitions though.

In HFSS CST EM simulation model we can't use Diodes.
Simple solution is to use Lumped RLC boundary in place of the Diode.
Co-simulation allows to import HFSS/CST model to be imported to the circuit simulator.
But how can we import circuit definitions(SPICE file etc.) to HFSS/CST simulation model?
In excitation option, right click, click on Edit Sources, in place of 1V or 1W, write any variable e.g. V1. It will ask for a value enter 1V/1W. Now when you click on model name in project manager, you will see variable V1 listed in property window, you can tune the value from here, you need not re-simulate every time you tune this variable.
For your other question Goto Modeler tab click on import, you can import many extensions find one that suits you.
You'd use equivalent scheme for varactor and, as you know C=f(Vbias), define C as a variable.
