Nalini...That has been the accepted practice among practitioners for years. Years ago, prior to the FDA approval of Kapvay, I tended to use the Clonidine patch on severely autistic children who were absolutely unmanageable early in the day on stimulants. Despite marked drowsiness for the first 1-2 hours, it was a powerful tool to decrease the unmanageable hyperactivity. I have found that the extended release clonidine is a far superior product not only for the autistic child, bur for the excessively hyperactive ADHD child who has been titrated to optimal effectiveness on a given long-acting stimulant. The Cmax ix not reached until about 4 hours, which eliminate the excessive drowsiness in over 50% of patients currently taking stimulants alone.
