Good response Brenda. Patresha - this seems a mute question to ask. It's pretty well known that there is a direct correlation between poor communication strategy and poor  quality care outcomes for patients. It's not 'rocket science' and any 'new' study would not reveal anything different to what is already well established. Poor practice = poor outcomes.

Wow.  Dean's right.  I'm surprised to find so much nursing included.  However, all you get are the citations and the abstracts; for most of the articles you'll have to go to CINAHL to actually get the article.

Not only are there articles, but there is a program put out by the Department of Defense called, "Team STEPPS," which is a way of overcoming communication difficulties.  Check it out at AHRQ.gov!

Effective communication requires listening and attention to the bio-psycho-social- and spiritual dimensions of the patient. Listening and presence are essential. The International Journal for Human Caring may be an excellent resource for caring communication.

I agree with Joan, maybe you should refer to Watson`s theory of caring moment, and nurse-patient interaction, how do we inculcate the culture of caring communication among nurses to improve the standard of nursing practice.

Given the theory of Jean Watson's Human Caring, I will point out a peculiarity that makes much sense to me in this theory - the emotional dimension in nursing practice. The emotions of caring must be perspetivadas and analyzed according to three aspects: 1) the emotional emperiência of processes associated with health-dioença customers; 2) the emotions experienced by nurses in the care process; 3) the emotions and feelings that they inhabit, and which are shared in the therapeutic nurse-client relationship. Regarding these three aspects, emotions have an influence on the effectiveness / ineffectiveness of communication in nursing.

Thank You guys for your interesting responses: This will be very helpful in my attempt to conduct a qualitative study. I have found that much study has been conducted on communication, however most seem to be using a Likert scale whereas I intend to use a pure qualitative design ( open ended questions); the purpose of which is to extract nurse perceptions and actual feeling on their experience with the phenomena which I hope will help to identify causes of ineffective communication not yet revealed and lead the way to the development of more effective nursing communication protocols. This topic has been a passion of mine for years and looking at the latest IOM data on sentinal events - it doesn't appear that an effective solution to the nursing problem is anywhere in sight. and since nurses are allowed to add their two cents   contribution to the future of nursing educational development I am sure excited for your gut comments. I am hereby attaching the instrument I have prepared for this study for your review and critique:. My intended focus group are RN's with at least 2 years of clinical experience. The attached  is a 15-20 minutes survey is geared at fulfilling my Capstone research requirement I sure would appreciate your kindness by way of participation - Thank you in advance or your time and participation 

Patresha - part of there confusion here is that you refer to your questionnaire as a 'survey questionnaire'. That usually relates to a descriptive quantitative design. You seem to want to post this 'interview' questionnaire out to participants and get them to offer you rich, narrative description from your questions. If you ask me - your approach is 'doomed to failure'. Most people put posted 'surveys' in the bin - unless they are quick-fire single response questions (closed - not open) - and not many of them. As soon as you ask for 'narrative' from open questions - without direct contact i.e. personal interviews, telephone interviews, or email interviews that are all directed, then you have no control - and the likelihood of 'rich' (or any) responses is extremely unlikely. 15-20 minutes is a very short time to conduct an interview - but a long time for someone without direction in a designated setting. 

Dean, I have to agree.  The tool may be overly ambitious.  Patresha, I think you should consider a Likert scale since you are taking a qualitative approach.

Most people doing research take the easy way out and most do arrive at the same conclusion. I am attempting to take a slightly different approach if that is ok with you. Thank you for your objectivity

Hi Patresha - it's not about 'taking the easy way out'; it's far more about what is practical and ensures that you don't end up 'wasting your time'. Being 'unconventional', especially for neophyte researchers, is not a wise thing to do. It's fine with me whatever you do - don't let me hold you back. I can't personally see how you will come up with a different conclusion - especially if nobody (or just one or two) responds and you have little or no data to analyse. 

I have used confidence scales with nursing students before and after communication skills teaching sessions; quantitative in nature, but as it was a representative sample we were able to produce some statistically significant data. I think capturing qualitative data requires a different approach such as using interviews. These days these can be conducted via Skype I believe.

I appreciate and am not underestimating your concern which I am sure it is based on accumulated experience. Please understand though, that I have never had the luxury of having anything done for me the easy  way in my life: This is a motivating factor for me to do my best always to achieve my desired end result. I am big on feelings and If there is some way I can get it accomplished I will by Gods Grace. Now it seems like I will have to work twice as hard to investigate and determine whether the quantitative approach works better than my intended qualitative approach. I am a nurse and have worked in several different areas of healthcare: MD clinic, ER, and med surg units. I have always focused on ensuring that both the patient and the familly/caregiver has a thorough understanding of their healthcare needs while at the same time trying to gather as much information as possible to investigate thoroughly the current medical crisis the patient is in. This takes time. Poor staffing, lack of time available, and needy families greatly impair this process.  I personally experienced this  when my father became very ill.  Although I wasn't always able to be present, I am well aware of my own family's overwhelming concern to help dad.  During recent hospitalizations,  I had 4 other siblings and my mom all firing questions at the Dr and nurses all at the same time despite my efforts to encourage them to allow dad to answer for himself and to allow the Dr and nurses to ask their own interview questions in order to gather the important facts of the current crisis. I was unsuccessful and, as a result, many important facts got missed (too many people at the admission and discharge process). The patient and caregiver education was minimal and dad ended up with repetitive hospitalizations. Ineffective communication has many causes. Once identified, improved communication can be achieved.  Working on identifying the barriers is part of the process. Improved staffing ratios; interviewing tools (such as interviewing algorithms); limited family presence (having one spokesperson?), would certainly help. I look forward to reading about your findings.
