Depends where it is the fistula ie anterior or hard palate or H-S palate junction. If it impairs speech then for sure it should be closed. If it is large amounts of fluids/food coming through the fistula one might consider the repair. Anterior fistulas should be address with placement of ABG.
in terms of best surgical techniques, I would suggest using large flaps, in need you could use a hemipalate flap to be advanced, otherwise it will break down. It should be tension free closure. If you have a post palatoplasty fistula, wait for at least 6 months to a year so tissues are softer again.
