i have recently had a neonatal MI at birth , troponin >50,000 .
there was ST elevation noted at birth .
any one who had similar experience .?
What troponin assay do you use? What is the unit for the 50,000? Any other symptoms in the kid? Cardiac function on echo Normal?
Not come across this although have seen MI in a three months old infant secondary to ALCAPA.
born at via normal delivery , term delivery , 41+1 week ,structurally normal heart .
troponin > 50,000.
Initial apgar of 0, 1 ,2 .normal coronaries.
no ALCAPA . ejection fraction was 0 .
resuscitated for 50 minutes before the heart rate came back from 0 --- 100. after 4 doses of adrenalin
echo showed biventricular failure . started on inotropes noradrenalin and adrenalin .
Decided to withdraw care due to ST Elevation , ventricular fibrillation and v tach as transport to a tertiary centre would have ended in an arrest.
active withdrawal care following prolonged discussion .

; mother had Protien C Deficiency . MI secondary to massive placental infarct .
Mum had a condition called developmental villous Immaturation.
