Has anyone a copy of Parent Motivation Inventory (Nock, 2006)(or other option to measure parent motivation to participate in a program)?

I am researching the effectiveness of a parent training and need to determine the baseline motivation level of the parents. I have read the article and it seems that PMI is most ideal. Unfortunately, I am unable to get it anywhere online.
