I have used resperidol quite a few times in patients suffering from post cardiac surgery agitation or delirium in a dose of 0,5 til 1 mg x 1 and 0,5 mg p.n that was very effective. Benzodiazepines were not very effective. One should always exclude other causes of agitation and delirium. Many cardiothoracic departments lack having psychiatry specialists and neurologist as part of the heart team. Having them as part of the team may help with prompt diagnose, treatment, research and experience in the fields.

In postoperative delirium in cardiac surgery, in patients with previous history of agitation or excitability, we have routinely used psychiatrist-recommended Seroquil safely and successfully at the Mayo Clinic In Rochester MN as well as later in my practice too.
