On June 30, 2013, 19 of the Precott FD Granite Mountain Hot Shot Crew (GMHS) were entrapped and died in a hastily constructed attempted "Safety Zone."
An ensuing Serious Accident Investigation Team (SAIT) concluded in their Serious Accident Investigation Report (SAIR) that they found (paraphrasing) 'no indication of violations of policy, protocol, reckless actions, or prodedure."' This "conclusion" followed a typical pattern of first establishing a "conclusion" and then supporting it with "facts."
The conventional wisdom is to use "facts" to lead one to a "conclusion" unless it is a wildland fire tragedy. Hence, we are learning "incomplete" lessons according to the Columbia and Challenger space craft accidents author and researcher Diane Vaughan.
There are several mentions of a firing operation along the Sesame Street and Shrine Corridor old dozer line road being improved by a dozer, to be used as a contol line for a subsequent firing operation.
There were as many as three separate firing operations by various WFs and FFs from local FDs.
There is considerable evidence of "separate and distinct smoke colimns (plumes)" from these firing operations, yet, to date, no real photos or videos of the actual firing operations.

Wildland firefighting deaths, like all work-rlated group deaths, are inevitable. All we can do is lesson or reduce them through "complete lessons learned."
