I'm looking for a survey or inventory, preferably a self assessment that looks at teachers instructional strategies in the classroom. I'd be very interested if it were based on Hattie's meta analysis.

It is a questionnaire for students to measure teachers' effective classroom behavior, yet it can easily be transformed into a self-assessment instrument and complemented with an observation instrument (see for that instrument: http://dx.doi.org/10.1080/09243453.2013.794845).
Both instruments are based on large-scale meta-analyses on effective classroom behavior, in line with Hattie's work. Let me know if you have any questions about these articles or the instruments, I'd be happy to reply!
