I want to measure whether and to what extent nurses report that they write a diary for the intensive care patient. I will use an already published questionnaire that has undergone face validity. I see that in order to get complementary enough answer options, I should add a "do not know" or "not relevant" to some of the questions. Is it allowed to do this? Do I then have to make a Cronb.alpha test?

Here's my humble take on your questions:
First, it depends on the license or permission provided by the authors of the original questionnaire (unless if it was you). in my opinion, at least we should inform them that you would be modifying the instrument.
Second, since it was modified, I highly suggest that you perform Cronbach alpha
