Elderly care is intrinsically interdisciplinary. I wonder if you could try search PubMed, Google Scholar, Scopus, etc with keywords of your choice.

Are you asking about nursing care, in-home care, medical care, emergency room care, rehabilitation care, social work care, etc? You mention the word "patients" in your question; what are they patients of? Have they had a fall? Do they have an acute medical condition? Are you referring to chronic conditions?

There's no specific field. It's in general wherever there are elderly receiving care from a interdisciplinary team. So any article focusing on the interdisciplinary collaboration while taking care of older adults will do.

There are certainly many resources out there. Have you googled "interdisciplinary care in older adults"? You should get a few results that way. Several show up that are in the Journal of American Geriatric Society (JAGS), American Journal of Nursing, and BMC Geriatrics. PubMed is also a great resource for such articles.
