I am conducting a questionnaire survey to collect valuable opinion about some significant aspects of museum. I therefore request you to kindly spare some of your precious time and express your sincere ratings and opinion on the under mentioned parameters given in the survey form. Prior of filling the survey form kindly provide your (Surveyee's) details. The survey form is divided into two parts:
Part – A, contains Fifteen (15) rating questions where each question can be rated on a scale of 1 to 5 [1 = Poor, 2 = Satisfactory, 3 = Average, 4 = Good, 5 = Excellent].
Part – B, contains Eleven (11) short subjective questions where each question can be answered in 2 – 4 lines (points only).
