Some Yoga Gurus says that practice of Kapalbhati is useful for all the practitioners whether they are Hypertensive or not. At the same time some Yoga experts says its not good for them.
I would not advice to do Kapalbhati or Bhastrika, or even Surya Namaskar, for somebody with tendency to hypertension; I actually observed one clinical case with a stroke very likely due to Surya Namaskar. One can argue that a proper technique would not cause adverse consequences, but it's hard to establish a proper technique. For an average practitioner, the answer is no.
I don't advice 'Kabhalabhathi' to any body; whether he/she is a hypertensive or not; because its purely unscientific owing to frequent and continuous excretion is against the natural low of human breathing.
Due to practice of kapalbhati, practitioner goes towards sympathodominance in hypertensive patients. Also it increases oxygen consumption, carbondioxide out put and energy cost very rapidly during the practice, which is very harmful for hypertensive patients. Thank you for taking interest in this question. More or less everybody suggesting to avoid Kapalbhati to the Hypertensive, but What is the mechanism of Kapalbhati, why it increases the Cardiac work load?
The Ancient Science behind it is that in hypertension there is a lot of subtle Tejas (fire) or physical Agni (fire) creating heat in the system, systemically producing swelling of the body, and because the head is the more delicate part of the body, it will receive more pressure, especially when overexerting and or bending downwards. As Mr Pal mentioned, at that time hypertension becomes sympathodominant, to use his words.
A more productive approach is to help people with hypertension or any other clinical image, become aware of the mechanisms that underlie the pathology. My research has led me to understand that mentioned mechanism is made up of a combination of three inner bio-behavioral responses that follow certain language-like patterns. When these become objectively visible they're referred to as body-language, emotional responses and coping stratagems. The physical symptoms are created by sustained bio-behavioral responses that are accompanied by contraction (fear-control)), burning (sadness-anger) and block (rejection-disassociation).
When sustained these responses create various symptoms according to one's hereditary genetic code that indicates one's sensitive areas that are more prone to the expression of imbalance and discomfort. In the case of hypertension the area of the Heart is more prone to mention responses. Hypertension in The Language of Emotions refers to an overwhelming sense of powerlessness and or frustration. The hyper tensed area is an expression of various compounding responses viz. tension (fear-loss of control), blocked (rejection-isolation) and heat (sadness-anger), which paralyzes. The theme always relates to earlier trauma in relating to others through the heart. And because the underlying pain is not recognized these protective mechanisms (fear, sadness and rejection) that normally keep the pain away from the conscious mind, continually influence one from beneath the conscious level, creating the symptoms of hypertension. When sustained, the individual progressively looses his/her dynamic capacity to seek balance.
Showing individuals through a system of "internal feedback" how these mechanisms work allows them to process what wants to be seen, facilitating integration on all levels and domains of life experience, from the inside-out. This leads to higher levels of cognitive, affective and psychomotor adaptation, which I refer to as "Inner Resiliency).
