The regression-based Reliable Change Index can be a good aproximation to interpret change across assessment waves. The problem here is that you need to have a sample to build the regression, and are limited to the test-retest interval analyzed in that sample. Besides that, you can analyze change using a univariate or a multivariate model including covariates for each individua with MCI.

Screening tests such as the MMSE, MoCA, or Clock Drawing will indeed fail to detect small changes, because they're designed to provide the minimum information needed to inform a diagnostic decision. What you want are tests that go into greater depth on specific functions, so there is enough potential variability in scores and so you can spot changes that affect only a single cognitive domain.

Here are a few suggestions: (1) the Coding subtest from the WAIS-IV; (2) the Digit Span subtest from either the WAIS-IV or WMS-IV; (3) the Spatial Span subtest from the WMS-IV; (4) the Rey Auditory Verbal Learning Test; (5) the Rey-Osterrieth Complex Figure; (6) a verbal fluency test such as FAS/Animals. If you are concerned about practice effects, you can easily make up different sets of digits or blocks for the two working memory tests, and there are alternative RAVLT lists and complex figures (some of these may be found in Lezak, I believe).

Hello Saeid, we are working in the same field and we find that dynamic testing (or learning potential assessment) is more sensitive than traditional testing in patients with MCI. We are currently working with the learning potential version of the Rey's Auditory Verbal Learning Test. We work with argentinean samples but you can find some of our works in english! I'll leave you some abstracts attached here, you can find the full texts, citations and other studies with this method in my profile!
