A 54 year-old man (H 187 cm and W 116 Kg; BSA 2.45 sqm) with bicuspid aortic valve (Vmax 234 cm/sec; Peak gradient 23,6 mmHg) with isolated dilatation of ascending aorta (52x51 cm). Risk factors: hypertension.

Do you place indication on elective surgery for ascending aorta replacement? Should the aortic index (Davies et al. Annals of Thoracic Surgery 81 (1): 169; 2006) be considered since he would drop into "low risk" category? Does this rule apply since he has a bicuspid valve?
