What is the best criterion for selecting the cutoff in a study on diagnosis, and using a cutoff on the ROC curve?

I want to know what criteria should be used when you do an analysis of ROC curves and gets results of sensitivity and specificity.Or will depend if we want screening criteria with my new test?
