I have 3 inserts for Golden Gate assembly. But every time I am doing it with NEB GG kit, 2 inserts are joining rather than 3. what can I do further?

1. I have checked internal restriction sites. 2. changed primers , kits, other buffers and enzymes, but it is giving me same result.
