What could be the reason for Zero reads in NGS Ion Torrent Gene STudio S5 raw data report for BRCA sample?

I am sure I have added the sample . So what could be the reason. I understand low quality reads but zero reads. Did anybody faced the same situation? Kindly help. Thanking you in advance.
