Can anyone tell me where I can get a full description of the rumination induction task (prompts, instructions etc) frequently used by Nolen-Hoeksema?

I need the description to use in my research. The task was first used by Morrow and Nolen-Hoeksema in 1993 but I have not manage to find a good description of the task yet.

Where can I get form B of the Disfunctional Attitudes Scale (DAS)?

I want to use the Disfunctional Attitudes Scale (DAS), forms A and B, in a study I am planning. I have form A but form B seems to be harder to find. Can anyone help me?
