Does anyone know of any child well-being survey instrument that accommodates the child's perception of his/her well-being?

I am carrying out a survey of children's well-being in a socially disadvantaged area. I am looking for a survey tool that allows for the voice of the child to be heard as recommended by the UNCRC....
