Where can I find official demographic questionnaire forms or samples that include ethnicity, education and income for several countries?

Hello everyone,

My team and I are conducting a cross cultural study and are currently drafting our demographics questions for our questionnaire.

I'm currently stuck on the answer choices for the ethnicity, education and income questions. The questions are 'what is your ethnicity' and the choices will be a list of ethnicities, 'what is your highest educational level?' and they will choose from a list of qualifications. Then the other question is 'What is your income?' (either weekly, monthly or yearly, we will decide based on how this was asked in each of the countries part of the study) with a list of income brackets to choose from.

All these questions will have different choices based on the participant's country which are the UK, Ireland, Sweden, Serbia, Cyprus (Turkish) and India. My question is where can I access official questionnaire forms or samples for these countries to help me draft my own questionnaire answers?
