When reviewing a recent article for a medical journal on a topic that was very familiar to me, I noticed a large portion of the paper, including the survey instrument, had been plagiarised. The authorship was hidden.
Despite my emailing the editor of the details of this obvious plagiarism and suggesting that the paper not only be rejected but also suggesting that the editor contact the authors institution to report this unprofessional and unethical behaviour; the paper was accepted and published in 2013. These instances tend to cast shadows over the integrity of some international journal editors and dilutes the credibility of peer review.
